How did you hear about us?

I understand Brazilian Jiu-Jitsu is a physical contact activity with inherent risk of injury. I confirm I (and any minors I'm registering, as their parent/guardian) am physically able to participate, and I agree to sign Rolling Dojo's liability waiver before the first session. * [Placeholder: have waiver language reviewed]

Thank you!

We got your intake. We'll reach out shortly to confirm your session, location and rate. See you on the mat.
